Typhoid fever commonly affects the liver, which is why evaluating blood count, typhoid antibody status, and liver function together gives your doctor a more complete picture of what the fever is doing to your body than any single test would.

The CBC gives your doctor a full picture of your blood cells. In a typhoid infection, the CBC often shows leucopaenia (a lower-than-normal white blood cell count), which is characteristic of the disease and helps distinguish it from bacterial infections where the WBC count is usually elevated. The platelet count may also drop during typhoid. The haemoglobin and red cell indices show whether anaemia is developing alongside the fever.
The Typhidot test detects IgM and IgG antibodies against Salmonella typhi, the bacterium that causes typhoid fever. IgM antibodies appear early in the infection, typically from the fourth or fifth day of fever, and indicate a current or recent infection. IgG antibodies appear later and may persist for months after recovery. A positive IgM result alongside consistent symptoms strongly supports a diagnosis of active typhoid. The Typhidot test is a rapid immunoassay that gives results within hours and is widely used in Indian clinics and laboratories.
The LFT measures a panel of enzymes and proteins that reflect liver health, including ALT (alanine aminotransferase), AST (aspartate aminotransferase), ALP (alkaline phosphatase), total and direct bilirubin, total protein, and albumin. Typhoid hepatitis, a recognised complication of typhoid fever, causes elevation of liver enzymes and sometimes bilirubin. Monitoring liver function alongside typhoid investigation allows the treating doctor to detect hepatic involvement early and adjust treatment if needed.
Typhoid fever is a systemic illness that does not stay confined to the gut. In a significant proportion of patients, particularly those with higher fever burden or delayed treatment, the infection spreads and involves the liver. A doctor managing a typhoid case therefore needs to know not just whether typhoid antibodies are present, but also how the liver is coping. Ordering the CBC, Typhidot, and LFT together in one blood draw avoids the delay and additional cost of sequential testing.
The combination is also useful when fever remains undiagnosed after initial screening. A normal Typhidot result alongside an elevated WBC count might shift suspicion toward a bacterial rather than typhoid-specific cause. Elevated liver enzymes without a Typhidot-positive result might indicate a viral hepatitis that needs further investigation. The three tests together help narrow the differential more effectively than any one of them alone.
Your doctor is most likely to order this combination if you have sustained fever lasting three or more days, particularly with associated headache, loss of appetite, abdominal discomfort, or coated tongue; if you are in or have recently travelled to an area with endemic typhoid; if preliminary blood tests have shown a dropping white cell count or platelet count that warrants further investigation; or if a previous typhoid test was inconclusive and you remain febrile.
This combination is also used for monitoring known typhoid patients to track treatment response and check for liver involvement during the course of illness.

If you have already taken antibiotics before the test, mention this to the phlebotomist and note it when sharing your results with your doctor. Early antibiotic treatment can occasionally reduce antibody titres and affect the Typhidot result.
