The CBC, Vitamin D, and Vitamin B12 combination is one of the most commonly ordered fatigue and deficiency workups in India. All three have overlapping symptoms when they are abnormal. Checking them together in one draw means your doctor can identify the cause of your fatigue, weakness, or bone pain without multiple separate visits and repeated blood draws.
This combination tests three distinct aspects of your health in one blood sample. The CBC evaluates your blood cells. The Vitamin D test measures whether you have enough of this hormone for bone strength and immune function. The Vitamin B12 test measures a nutrient essential for red blood cell production and nerve health. Together, the three results allow your doctor to distinguish between different causes of the same symptoms and tailor treatment precisely.
The CBC measures your red blood cells, white blood cells, and platelets. In the context of this combination, the most clinically relevant findings are haemoglobin and the red cell indices (MCV, MCH). Vitamin B12 deficiency causes megaloblastic anaemia, where red blood cells become abnormally large (high MCV). Iron deficiency causes small, pale red cells (low MCV, low MCH). The CBC therefore tells your doctor which type of anaemia, if any, is present, which in turn points to the underlying deficiency.
The 25-OH Vitamin D test measures the storage form of Vitamin D in your blood, which reflects your overall Vitamin D status most accurately. Vitamin D plays a central role in calcium absorption and bone mineralisation, immune regulation, and muscle function. Deficiency is extremely common in India, affecting a large proportion of urban adults due to limited outdoor sun exposure, darker skin pigmentation, and dietary gaps. A level below 20 ng/mL indicates deficiency. A level between 20 and 30 ng/mL is insufficient. Above 30 ng/mL is generally considered sufficient. Symptoms of deficiency include bone pain, muscle weakness, fatigue, and increased susceptibility to infections.
Vitamin B12 is essential for the production of healthy red blood cells, the maintenance of the myelin sheath that protects nerve fibres, and the synthesis of DNA. Deficiency develops slowly, particularly in people following a strict vegetarian or vegan diet (B12 is found almost exclusively in animal products), in older adults with reduced stomach acid, and in patients taking certain medications such as metformin. Symptoms include fatigue, tingling or numbness in the hands and feet, weakness, difficulty concentrating, and mood changes. Severe deficiency causes irreversible nerve damage if not treated promptly. The normal serum B12 range is approximately 200 to 900 pg/mL, and levels below 200 pg/mL are considered deficient.
Fatigue and weakness are among the most common symptoms in outpatient practice. The cause could be anaemia from iron deficiency, anaemia from B12 deficiency, the generalised tiredness of Vitamin D deficiency, or a combination of all three. Because these conditions can coexist and produce the same surface-level symptoms, testing for them simultaneously gives your doctor the complete picture.
The CBC adds specificity. A low haemoglobin with a high MCV strongly suggests B12 deficiency anaemia and should prompt B12 testing. A low haemoglobin with a low MCV points to iron deficiency. A normal CBC in someone with fatigue and bone pain shifts suspicion toward Vitamin D deficiency without anaemia. The three tests together save the time and cost of sequential, stepwise testing.
This combination is appropriate for adults experiencing unexplained fatigue, weakness, or lethargy that has lasted more than a few weeks; patients with tingling or numbness in the hands or feet; strict vegetarians or vegans who have not supplemented B12; older adults with reduced appetite or absorption; patients who have been on metformin or proton pump inhibitors long-term; people with limited outdoor sun exposure; women of reproductive age who are planning a pregnancy (Vitamin D and B12 status matters for foetal development); and anyone whose doctor has flagged a possible deficiency on previous tests and wants a fresh assessment.
Fasting is not required for the CBC, Vitamin D, or Vitamin B12 test. You can eat, drink, and take your regular supplements and medications before the phlebotomist arrives. If you are currently taking Vitamin D or Vitamin B12 supplements, mention this to your doctor when reviewing the result, as active supplementation will raise the measured level. The test is most informative when taken before starting supplementation, or at least four to six weeks after stopping it if your doctor wants to assess your baseline.
