The CBC + Urine Routine is a combination of two fundamental diagnostic investigations: a blood test that analyses your blood cells, and a urine examination that assesses the physical, chemical, and microscopic properties of your urine. The two tests complement each other because many conditions that affect the blood also leave evidence in the urine, and vice versa.
The CBC analyses red blood cells (count, haemoglobin, haematocrit, and indices such as MCV and MCH), white blood cells (total count and differential showing the proportions of neutrophils, lymphocytes, monocytes, eosinophils, and basophils), and platelets. It is used to detect anaemia, infections (bacterial, viral, or parasitic), inflammatory conditions, clotting disorders, and blood cell abnormalities. It is one of the most informative basic tests available and is almost always included in any initial workup.
The Urine Routine (also called a urine routine and microscopy, or URM) examines your urine in three ways. Physical examination looks at colour, clarity, and specific gravity. Chemical examination uses a dipstick to detect glucose, protein, ketones, blood, bilirubin, urobilinogen, nitrites, and leukocyte esterase. Microscopic examination counts red blood cells, white blood cells, epithelial cells, bacteria, casts, and crystals in a spun urine sediment. Together, these findings tell your doctor about the health of your kidneys, bladder, and urinary tract, and can also reflect systemic conditions such as diabetes (glucose in urine), liver disease (bilirubin), and dehydration (high specific gravity).
Several common clinical situations benefit from reviewing both the CBC and the Urine Routine simultaneously. In a fever workup, the CBC shows whether white cells are elevated (suggesting infection), while the urine tells the doctor whether the infection might be in the urinary tract. In a patient with fatigue, the CBC identifies anaemia and the urine shows whether protein loss through the kidneys (nephrotic syndrome) is contributing. In a patient with suspected kidney disease, the urine microscopy is central but the CBC shows whether anaemia of chronic kidney disease is already present.

The urine sample should ideally be a midstream morning specimen, collected at the time of the phlebotomist's visit. To collect a midstream sample, start urinating, then collect the middle portion of the urine stream in the sterile container the phlebotomist brings. This technique reduces contamination from skin or genital bacteria.
Drink enough water in the morning to be able to provide a urine sample when the phlebotomist arrives, but avoid excessive over-hydration as this can dilute the sample and affect the specific gravity and some chemical results. Women should inform the doctor if they are menstruating, as blood contamination of the urine sample can affect interpretation.
